  • General physical exams, preventative care, and vaccinations.

  • Screening and treatment for high blood pressure, asthma, high cholesterol, diabetes, thyroid and other chronic diseases.

  • Guidance and medical treatment for weight management and smoking cessation.

  • Routine women's health care, from breast exams, pap smears to osteoporosis prevention and treatment.

  • Acute care for urinary tract, upper respiratory and other infections.

  • Travel counseling and vaccinations.

  • A wide range of lab tests, from basic blood and urine tests to screening for HIV and Hepatitis, and EKGs, all performed on site.

  • Evaluation for anxiety and depression.

Policies

  • Financial Policy: You will be asked to sign our Financial Policy at check-in. It can be accessed by clicking Financial Policy.   If you have questions about your bill please see your insurance EOB and contact your insurance carrier.  Also, our billing company contact number is 347-732-1357 or you can message billing on our patient portal.

  • Prescribing Medications: If we prescribe a recurring medication at your visit, we will ask you to schedule a follow-up visit for additional refills. Set this visit up prior to leaving the office or at least 1 week prior to running out of medication. Refills cannot be continued if you have not come for a follow-up visit. Controlled substances are only prescribed during an office visit in accordance with NYS law. All new prescriptions also require an office visit.

  • Patient Portal: Medical practices are now using electronic means to communicate with patients. This allows patients to request appointments, send non-urgent messages and review clinical data. We also use the portal to message you about appointments, results and follow-up. We require that you sign up for the portal when registering and check any messages or notifications promptly. Please do not use portal messaging for urgent or new medical issues, as messages are only checked during Office Hours. Instead, please call the office to schedule an appointment with a provide

  • Referral Requests: If your insurance requires a referral or pre-authorization to see a specialist or schedule a procedure, please notify our front desk on portal or voicemail at least 2 business days prior to your appointment for processing. We will need your information, the specialist’s information (insurance ID/fax number), and the date/reason for the visit. If your doctor has not ordered the referral, we will need to have you come in for an appointment.
